  • Overview of Client

  • Current Situation

    Working in partnership with the Metro South Addiction Mental Health Services (MSAMHS), a new Transitional Recovery Accommodation service has been established. This innovative new service is designed to support adults with mental health needs as they transition from hospital back into their community, home and/or community housing.   

    This is a pilot program, funded for a period of 3 years through to 30 June 2027.  Since receiving the funding in September 2024, the first residential step-in house has opened in Greenbank offering live in high intensity support. Two other houses offering low intensity live in support and wrap around services to assist with integration back into the community will come online in April & June 2025.  

    The role also oversees the Support and Sustain program, providing another pathway for up to 4 clients per year who can remain in their own home but need support using a case management model. 

  • Role Description

    We are looking for a skilled and competent leader who will be proactive, engaged and physically present in the service.   Key stakeholder engagement is central to the model – outward facing with great community awareness – and a passion for the opportunity this new initiative provides to improve the lives of individuals through individualised recovery-oriented support.  

    Karakan is championing this pilot program and will leverage from the model and its outcomes – showcasing in action the work that is being done.  You must be a people person who is curious and open to acting as an advocate and strong voice for the service and the clients. 

  • Staffing Profile

    A strong leader is needed to mentor, engage, supervise and motivate the staff to bring them along the client journey and build a vibrant and inclusive team culture. The funding presents a unique opportunity to build a new service from the ground up, putting all your passion, skills and experience to work.   

    When fully recruited, the team includes 2 x Case Managers and 10- 12 Recovery Workers working in partnership with a professional employed in the MSAMHS team – fully committed to making this service a success. The first Case Manager has been employed with the second due to come on board at the end of May 2025. The successful candidate will be involved in the recruitment process for the second role. 

  • Why this role?

    While a greenfield service, established systems, processes and structures are in place, that work has been done.  We need a strong leader to build the culture, manage the program and bring the multidisciplinary team together, working from an understanding of mental health and recovery-oriented practice.    Karakan values diversity, a commitment to support and empower others and lived experience.

  • Requirements
    • National Police Check 
    • Working with Children/Blue Card 
    • NDIS Worker Screening 
  • Qualifications (as per Position Description)
    • Bachelor’s degree in psychology, social work, occupational therapy, human services, or a related field 
    • Minimum of 3 years of experience in leading/coordinating mental health services, ideally a community-based recovery-oriented accommodation service or step-up, step-down service
