As a Service Leader, you’re a skilled and compassionate case manager focused on best practice with the drive and purpose required to enable and empower clients to achieve their individual goals.
You’re both a conduit and a voice. Clients and families will look to you for advice and support in tough times. Your compassion will shine through as you partner up to understand, assess, plan and support each and every client through their individual health care journey. With an eye on the ball, you will assist clients and those around them to access the right care and support they need.

You are a strong leader who can provide mentoring, supervision and support to build the capacity of your community-based team – deeply embedded within the community, ensuring clients and families receive the best care and support possible.
Candidates must have a Diploma in Community Services/Disability or equivalent or relevant industry experience, along with Blue and Yellow Cards. Demonstrated experience in a leadership role with strong coordination, case management, problem solving, leadership and peer support skills will ensure your success. The ability to listen, negotiate and work within regulatory frameworks is essential.
