Work with the management team to establish and deliver Clinical Care Coordination services for Hervey Bay.

As a Clinical Care Coordinator, you’re a pivotal part of a healthcare orchestra—collaborating to ensure that all the instruments (or in this case, multidisciplinary mental healthcare professionals) play in harmony to create a beautiful symphony of client care.

In this intricate dance of mental health care, your pen waltzes across care plans, stitching hope into every line. You’re the bridge between client, families, and the entire healthcare ensemble. Your inbox is a bustling marketplace of emails, voicemails, and carrier pigeons (okay, maybe not pigeons). You ensure everyone’s on the same sheet of music.

You’re part therapist, part cheerleader. Clients and families lean on you during tough times. Your empathy is a soothing balm. Your work isn’t just about charts and checklists, you weave compassion into the very fabric of healthcare. Like a gardener tending to delicate blooms, you monitor client progress. If the treatment plan needs tweaking, you adjust it. Flexibility is your superpower.

This is a clinical position. Candidates must have a tertiary qualification or equivalent in Mental Health or a related discipline. Experience or knowledge of the Mental Health, Alcohol and Other Drugs (AOD) and Suicide Prevention sector and an understanding of client needs are essential. A minimum of three years’ experience is required. Accredited Mental Health Clinicians (such as Psychologists, Social Workers or Mental Health Nurses) are preferred due to their expertise in providing therapeutic support. AHPRA registration or membership with AASW is essential.
