Our client is on the lookout for a compassionate and capable Care Coordinator to join their team on a 2-month assignment, starting immediately. This is a fantastic opportunity to make a meaningful impact supporting older Australians through the Support at Home Program.
You’ll be coordinating care for around 60 clients across Hervey Bay, Moreton Bay, and Brisbane, ensuring services are delivered with empathy, professionalism, and in line with evolving legislation. Your day-to-day will include:
- Responding to client queries about care plans, budgets, and entitlements
- Supporting onboarding of new clients and managing incoming leads
- Educating clients and providers on rights and responsibilities under new legislation
- Liaising with associate providers to ensure care standards and compliance
- Monitoring client needs and ensuring timely, appropriate service delivery
- Interpreting and applying legislative changes during the program transition
- Providing internal guidance and ensuring documentation meets compliance standards
- Handling complex conversations with clients and families
- Engaging with clients in person, particularly in Hervey Bay
- Maintaining accurate records of client interactions and care plans
- Answering phone calls and emails related to care coordination
You’ll bring a strong background in home care package management, ideally from an organisational rather than service delivery perspective. You’ll also have:
- Qualifications in Nursing, Occupational Therapy, Social Work, Psychology, Speech Pathology, or other Allied Health fields
- Strong literacy, numeracy, and analytical skills
- Confidence in navigating complex conversations and legislative frameworks
- Familiarity with dignity of risk, client rights, and care governance
The clients include older Australians, some with ICare packages and total disabilities, so empathy, professionalism, and a genuine commitment to quality care are essential.
